Please submit this sheet at least 2 weeks prior to the requested start date for the Birthday Wish ( ie. If you want it to start January 14, submit this sheet by January 1).
Payment must be received with the Birthday Wish order form. You will receive your Chacko's Family Bowling Center tee- shirt and birthday card at an arranged date between you and Kara Chacko.
Your wish will start on the date specified above when Chacko's Family Bowling Center opens and will run continuously for one full week.
Please note that ScreenSaver Birthday Wishes only run on the screens when lanes are not in scorekeeping mode.
